It is called a vitamin, but that label is technically wrong. Vitamin D is a hormone. Or more precisely, it is a prohormone, a compound the body converts into an active hormone that reaches nearly every tissue and cell you have. It participates in regulating mood, immune function, bone metabolism, cardiovascular health, muscle performance, and the expression of thousands of genes. It is manufactured by the skin in response to ultraviolet B radiation from the sun. And despite the fact that we are now deep in the height of summer, when sunlight is most abundant and most potent, the majority of adults in the developed world are not getting enough of it.

Understanding why requires stepping back from the conventional idea of vitamins as simple supplements and looking instead at what this compound actually does once it enters the bloodstream. The picture is far more complex, and far more important, than most people realize.

A Hormone That Governs Far More Than Bone

When vitamin D enters the circulation, the liver converts it into an intermediate form called calcidiol. The kidneys then convert calcidiol into calcitriol, the active hormonal form. From there, calcitriol binds to vitamin D receptors located throughout the body, and those receptors are found in an extraordinary range of tissues: the brain, the heart, skeletal muscle, the pancreas, the immune system, the colon, and the skin itself. Each tissue has its own local use for the compound, which is part of why vitamin D deficiency can manifest in such diverse and seemingly unrelated ways.

The most widely known function is bone health. Vitamin D regulates the absorption of calcium and phosphorus in the intestine and plays a direct role in bone mineralization. Without adequate vitamin D, calcium absorption drops significantly, and the body is forced to draw calcium from the skeleton to maintain blood levels, a process that, over time, weakens bone density and contributes to conditions like osteoporosis and osteomalacia. This connection is well established and has been understood for decades.

What is less commonly discussed is what else vitamin D is managing at the same time. Research over the past two decades has substantially expanded the picture. Vitamin D plays a regulatory role in the adaptive immune system, influencing both the innate immune response and the body's tolerance mechanisms that prevent autoimmune overreaction. Low vitamin D has been associated in multiple large studies with higher susceptibility to respiratory infections, including influenza and certain upper respiratory tract infections. It appears to support the production of antimicrobial peptides, compounds the immune system uses as a first line of defense against pathogens.

The relationship between vitamin D and mood is also well documented. Vitamin D receptors are present in regions of the brain involved in mood regulation, including the hippocampus and the prefrontal cortex. The hormone appears to influence the synthesis of serotonin and dopamine, two neurotransmitters central to motivation, emotional stability, and the experience of reward. Epidemiological studies consistently find associations between vitamin D deficiency and higher rates of depression and seasonal affective disorder. Whether the relationship is causal, and in what direction, remains an active area of research, but the overlap is too consistent to dismiss.

Why Summer Does Not Guarantee Sufficiency

There is a widespread assumption that spending time outdoors in summer is enough to maintain healthy vitamin D levels. The truth is more conditional than that. The skin's ability to synthesize vitamin D from sunlight depends on a specific wavelength of ultraviolet radiation, UVB, and that wavelength is only present in sunlight when the sun is high enough in the sky. At latitudes above approximately 35 degrees north, which includes most of the continental United States, meaningful UVB exposure is limited to a window of midday hours during the warmer months. Early morning and late afternoon sun, even in peak summer, produce almost no vitamin D synthesis regardless of time spent outdoors.

Sunscreen further complicates the picture. An SPF 30 sunscreen, applied properly and consistently, reduces UVB penetration to the skin by approximately 95 percent. This is enormously effective at reducing UV damage and skin cancer risk, both of which are legitimate and serious concerns, but it also means that sunscreen use, while important, substantially limits vitamin D production. Glass blocks UVB entirely, which means time spent near a window or in a car does not contribute to synthesis. Even clothing plays a role: any fabric covering the skin prevents the reaction from occurring in the area it covers.

Skin tone is another important variable. Melanin, the pigment that determines skin color, acts as a natural UV filter. People with deeper skin tones require significantly more sun exposure to synthesize the same amount of vitamin D as those with lighter skin. This is one reason why vitamin D deficiency rates are disproportionately high among people of African, South Asian, and Middle Eastern descent living in temperate climates. Body composition also affects the equation: vitamin D is fat soluble, meaning it is stored in adipose tissue, and in individuals with higher levels of body fat, a greater proportion of circulating vitamin D may be sequestered in fat stores and unavailable for use.

The Deficiency Numbers Are Not Small

The Endocrine Society defines vitamin D sufficiency as a serum 25-hydroxyvitamin D level of at least 30 nanograms per milliliter. By that standard, an estimated 40 percent of American adults are deficient, and a substantially larger percentage fall into the insufficiency range, which carries its own set of functional consequences even when full deficiency is not reached. Globally, vitamin D deficiency is one of the most prevalent nutritional shortfalls across all age groups and geographies, including sunny climates. The numbers suggest that this is less about inadequate sun and more about the cumulative effect of modern indoor lifestyles, sunscreen habits, and dietary patterns.

The consequences of prolonged deficiency extend well beyond bone loss and fatigue, though those are often the most visible signs. Research has associated low vitamin D with increased risk of cardiovascular disease, type 2 diabetes, several autoimmune conditions including multiple sclerosis and rheumatoid arthritis, and certain forms of cancer, particularly colorectal cancer. These associations do not establish simple causation, and vitamin D supplementation alone has not been shown to reverse disease risk in controlled trials as dramatically as observational data might suggest. But the consistent directionality of the evidence across many conditions points to a compound that is doing something fundamental in the body's regulatory infrastructure.

Supporting Your Levels Intelligently

The most reliable way to know where your levels stand is a simple blood test. A 25-hydroxyvitamin D test, available through any standard lab panel, gives you a clear number to work from. This matters because the symptoms of deficiency, including fatigue, low mood, muscle weakness, and diffuse aches, are nonspecific and easy to attribute to other causes. Many people spend years assuming their energy or mood issues are purely behavioral when a simple deficiency is contributing meaningfully to how they feel.

Dietary sources of vitamin D are limited but worth including. Fatty fish such as salmon, mackerel, and sardines contain meaningful amounts. Egg yolks, beef liver, and certain mushrooms that have been exposed to sunlight also contribute. Many foods are fortified with vitamin D, including dairy products, plant-based milk alternatives, and certain cereals, though the amounts added through fortification are generally modest. For most people who are genuinely deficient, supplementation under the guidance of a physician is the most direct path to correcting levels. Vitamin D3, the same form the body produces from sunlight, is generally considered more effective at raising serum levels than D2.

Sensible sun exposure remains valuable, not as the sole strategy, but as part of an integrated approach. Ten to twenty minutes of midday sun exposure on arms and legs without sunscreen, a few times per week during summer months, can produce a significant amount of vitamin D in those with lighter skin tones. After that initial exposure, applying sun protection makes sense. The goal is not to avoid the sun entirely but to use it thoughtfully, understanding what it can and cannot provide.

The Broader Lesson

What vitamin D illustrates more than almost any other compound is how profoundly the body depends on environmental inputs that modern life systematically reduces. We moved indoors. The body was designed to interact with sunlight, with the ground beneath our feet, with seasonal rhythms and physical rest. When those inputs are removed, the effects are rarely dramatic at first. They show up gradually, in energy that never quite returns, in mood that stays slightly flat, in a nervous system that cannot fully settle.

The vitamin D story is also a reminder that wellness is not primarily a matter of adding things to the body. It is largely a matter of restoring what the body has always needed but has been quietly denied.
